COMMUNITY HEALTH STORIES: LOCAL HEALTH HEROES

Local Health Heroes is an evening of storytelling and sharing about community health innovators and initiatives. Join us to hear from local community health heroes who are:
- Improving access and tailoring health services to meet the needs of immigrants and refugees;
- Enabling seniors to get care at home that would normally require a visit to the doctor;
- Working in a primary care setting to provide patients with access a full range of health care providers from physicians and pharmacists to nursing and peer support;
- Supporting people with disabilities to advocate for their health care needs, plan for their future and connect with the broader community;
- Providing housing and health services to people who are homeless and/or living in poverty in ways that work for them; and
- Empowering women prisoners to participate in a research process that helps us understand how to address their health challenges.
